  • We offer all recommended & required vaccines for children and adults

  • We accept all Medicaid plans, all Medicare plans, and most private healthcare plans for billing

COVID-19 VACCINATION CLINICS:​

Pfizer COVID-19 vaccines are available by walk in on Mondays from 9:00 a.m. to 12:00 p.m. and 1:30 p.m. to 4:00 p.m. If you need an appointment at a different time, please call the nurse to schedule at 440-992-7122.  Remember to bring your insurance card with you for your visit.

CDC COVID-19 Information

Mantoux testing (tuberculosis skin test)

  • $10.00 (cash, check or credit card only)

  • must return 48 hours later for reading of skin test.

Children with Medical Handicaps Program

  • The CMH program is administered by the Ohio Department of Health.  The program serves children with special health care needs and their families.  The goal of the CMH program is to assure that children with special health care needs and their families obtain comprehensive care and services.  Visit the Ohio Department of Health website for more information on the program.  www.odh.ohio.gov & type BCMH in the search box.
