We have ONLY Ashtabula City death certificates

The Ashtabula City Health Department can issue birth records for anyone born in the State of Ohio.  Death records can only be issued for persons who dies in Ashtabula City from about 1909 to the present.  You can come into the office Monday through Friday between 8:00am-12:30pm or 1:30pm-4:30pm to obtain a certified copy of the desired certificate.  A certified copy of a birth or death certificate has the raised seal and the signature of the Registrar of Vital Statistics.  The cost for a certified copy is $25.00.  Uncertified copies are no longer available according to the Ohio Revised Code.

  • You may walk-in and obtain your birth or death certificate in about 5 minutes.
  • You may mail in your request, use the birth or death certificate request link down below to see what information is required.
  • You may use the birth and death certificate request form & fax your information to us at 440-992-7163.


Genealogy Search

We are glad to assist you in your search for your family genealogy.  All we need are the persons’ names and approximate dates of birth or death and what information you are searching for.  We will search our files and allow you to view the documents related to your request.  While all death records are open for review, birth records are limited due to the confidential health information they contain.  Unfortunately genealogy requests can not be addressed over the phone, you are welcome to come to the office directly, email or mail a written request to us.
