Our new clinic hours due to COVID-19 are Mondays - Fridays 8:00am-12:30pm & 1:30pm-3:30pm by appointment only

Please note: All patients over the age of 2 years will be required to wear a mask and pass a screening questionnaire to be seen by the nurse.  One caregiver per patient is allowed.

    • We accept all Medicaid plans, all Medicare plans, Anthem Blue Cross & Blue Shield, Cigna, Medical Mutual and Summa Care and Humana for billing
  • Mantoux testing (tuberculosis skin test

    • must return 48 hours later for reading of skin test.
  • Blood Pressure Screening
    • Mondays-Fridays by appointment at no charge

  • Children with Medical Handicaps Program
